DETAILS ABOUT A0 FORM (WITH LINKS FOR ELECTRONIC VERSION!)
- A0: The MESA parent permission form. The signed document comes back to Regional Coordinator for entry in Thinkwave. Electronic is best, but you can also print hard copies and send back to RC (can be faxed or emailed)
- Electronic A0 – Parents can fill out the whole form online, then it is sent to me and I can keep track of it all. You don’t need to do anything other than ensure that students do it. ALSO, MAKE SURE STUDENTS KNOW THEY’RE IN THE NORTH CENTRAL REGION. For those interested, here’s the A0 QR code

- A-3: Field Trip form
- Electronic A3 – Also easier to do it all electronically. When it is completed by a student, I will receive a copy. Required for all students going on a MESA field trip. For those interested, here’s the A3 QR code.
