StudentsLoyalty Award Application


Before you begin this application, you must have the following items readily available. You cannot save and return to a partially completed application at a later time.  You will be asked to resubmit if incomplete. 

  • A valid email address that will be active until the end of the current calendar year (DO NOT USE A HIGH SCHOOL EMAIL)
  • An electronic copy of a completed federal W-9 form (complete, save and download a copy of the form) – MUST INCLUDE A SIGNATURE
  • An electronic copy of a personal testimonial about the impact that MESA has had for you.
  • A saved version of your college schedule –THIS MUST INCLUDE the School name, Your Name, Your Student ID#, and your classes.  It must be something generated from the school. 

This site is no longer accepting appeals for the Loyalty Award for 2022. If your 2022 submission was complete Loyalty Award Checks will be released on September 26. Any questions, please contact
