Before you begin this application, you must have the following items readily available. You cannot save and return to a partially completed application at a later time. You will be asked to resubmit if incomplete.
- A valid email address that will be active until the end of the current calendar year (DO NOT USE A HIGH SCHOOL EMAIL)
- An electronic copy of a completed federal W-9 form (complete, save and download a copy of the form) – MUST INCLUDE A SIGNATURE
- An electronic copy of a personal testimonial about the impact that MESA has had for you.
- A saved version of your college schedule –THIS MUST INCLUDE the School name, Your Name, Your Student ID#, and your classes. It must be something generated from the school.
