- Order Form Should be Submitted by Teacher Advisor to anita@nmmesa.org by October 20, 2021 at 5PM – Late Forms will not be honored.
- Students will be verified and MUST have their A1 and A2 submitted by October 14 – The same deadline for Activity Checks.

General Information:
- An advisor will be provided a shirt for every (10) eligible students. Advisor shirts are only provided to official school advisors.
- Shirts can be customized after (20) eligible students. Otherwise shirts will be Dark Grey with white MESA logo. No School Name will be printed on the shirt.
- Shirts are planned to be basic Gildan Unisex Tees in Youth (Y) and Adult (A) sizes. NM MESA reserves the right to substitute as needed for similar color/shirt (you will be notified if needed) based on supply constraints.
- Customization includes:
- Selection of Shirt Color from listed colors on order form
- Selection of Ink Color from listed colors on order form
- Addition of school name above/below logo as listed on order form
