Before you begin this application, you must have the following items readily available. You cannot save and return to a partially completed application at a later time. This deadline has been extended to May 1st due to current circumstances with Covid-19.
- A valid email address that will be active until the end of the current calendar year
- An electronic copy of a completed federal W-9 form (complete, save and download a copy of the form.) If your Regional Coordinator already has your W-9 then just upload a blank document to complete the submission.
- An electronic copy of a personal testimonial about the impact that MESA has had for you.
The form for submitting an application for the NM MESA Loyalty Award will be available on this page March 15-April 16 2021. You must have the items described above to submit the application.
